- File appeal/request for administrative fair hearing within 30 days of the contested action (period begins 5 days after the mailing date) - 210-RICR-10-05-2.2.1(A)(9)
- Continuation: file within the 10-day advance notice period (10 days beginning on the 5th day after the notice date)
- Managed care: exhaust the plan's grievance and appeal process first
- After exhausting the plan appeal: request an EOHHS fair hearing within 120 days (Section 2.4.2(B))
