- File written fair-hearing request with SCDHHS Division of Appeals and Hearings within 30 days of the notice (S.C. Code Regs. 126-152(A))
- DD/ID waiver (DDSN): appeal to SCDHHS within 30 calendar days of the DDSN notification (Directive 535-11-DD)
- Continuation: submit the fair-hearing request within 10 days of the notice date
- Managed care (Healthy Connections MCO): file plan appeal within 60 days, then request State Fair Hearing within 120 days of the Notice of Resolution
- Judicial review: appeal to SC Administrative Law Court within 30 days of the hearing officer's order
