Call your county's Community Mental Health Services Program and ask for an intake and eligibility screening for developmental disability services. Do not frame this as applying for the Habilitation Supports Waiver alone — under Michigan's Mental Health Code, CMH has an obligation to serve people with developmental disabilities within available resources, so ask what services you can have now, then ask separately to be screened and scored for the HSW.

WHO TO CONTACT, IN ORDER
Five different offices, and they don't talk to each other. Disability services, health coverage, and income each run through a separate agency — applying to one does not start the others. Ask each office for the name of the person you spoke to and write down the date; if something is later denied, that record is what an appeal is built on.

Developmental disability services
This is the office that decides whether your child is eligible for disability services. Ask for "intake" or "eligibility determination" by name. In Michigan, you contact Community Mental Health Services Program (CMHSP).

Health coverage — Michigan Medicaid (Healthy Michigan Plan)
Almost every waiver and personal-care route requires this first, and it is decided by a different office from the one above. Apply in parallel rather than waiting.

Help at home, and who can be paid for it
Unless you say otherwise, services come the traditional way: the agency picks and pays the providers. Self-direction — where you get a budget and hire your own people — is opt-in almost everywhere and is not offered automatically. Here it is called Self-Determination / Choice Voucher. Ask for it by that name.

Income — SSI
Federal, so it's the same everywhere: 1-800-772-1213 (TTY 1-800-325-0778). There is no minimum age — you can apply for a newborn. Until 18, the parents' income is deemed to the child, so an early denial is usually about that rather than the child's disability. In many states an SSI award also opens Medicaid automatically — ask when you call.

Every state has a Protection & Advocacy agency by federal law — free legal help on disability services and appeals. Most families never hear of it, and it is the number to have before a deadline, not after. Yours is Disability Rights Michigan.

In many states you can control the budget and hire your own people — including, sometimes, family. Most families are never told this is an option. Here's what we've verified for Michigan; where we haven't verified an answer we say so — it does not mean the answer is no, and it is worth asking.

Who is legally the employer of the people you hire?
Three models, and which one you pick decides who the legal employer is. Direct Employment: you are the employer and hire your own staff, with a Fiscal Intermediary handling the money. Agency with Choice (agency-supported): a provider agency is the employer and you decide how much of the managing role you want. Direct Contract: you contract with a professional directly under a Purchase of Service Agreement, setting wages and terms within the service rate. Michigan runs self-determination through your local CMHSP/PIHP, so confirm which models yours offers.
How freely can money move between services?
Your budget is built from the ground up rather than handed to you as a fixed pot — MDHHS uses a retrospective zero-based method, costing out the services in your plan. There is a standing review cadence you can use: you and your supports coordinators revisit the budget whenever the need arises, and in any case at least every 90 days. Ask for a review rather than waiting for the annual plan if something is not working.
Can the budget buy things, not just staff time?
Partly, and not as a separately named purchase service in what was checked. Michigan's self-direction guidance describes the individual budget as giving you latitude to buy individually selected goods and services, not only to employ workers — but no distinct Individual Directed Goods and Services line item like New York's or Ohio's was found. Ask your CMHSP what non-staff purchases your budget will cover and how they must appear in your IPOS.
